Blood results normal but still feeling off? Here’s what I look for as a clinical nutritionist

“My blood tests came back normal… so why do I still feel awful?”

If you’ve ever asked yourself this question, you’re certainly not alone.

In fact, it’s one of the most common reasons people book an appointment.

Many of my clients arrive feeling frustrated after being told that their blood test results are “normal.” Yet they’re still dealing with fatigue, bloating, brain fog, hair loss, anxiety, stubborn weight gain, irregular periods or digestive issues.

They’re beginning to wonder if it’s all in their head, but the truth is, it usually isn’t.

As a Clinical Nutritionist, I don’t diagnose disease – that’s your doctor’s role. But I do spend a lot of time helping people understand blood test results from a functional perspective. Instead of asking, “Is this number inside the laboratory range?” I ask a different question:

“Is this result optimal for this person’s health, symptoms and goals?”

Sometimes the answer is yes, but often, the answer is no.

Why Normal doesn’t always mean healthy

One of the biggest misconceptions about blood tests is that normal automatically means healthy.

Laboratory reference ranges are designed primarily to identify disease or significant abnormalities.

They’re incredibly valuable, but they don’t always tell us whether your body is functioning at its best.

For example, someone can technically fall within the laboratory’s reference range while still experiencing symptoms related to nutrient deficiencies, inflammation or poor thyroid function.

This is why understanding blood test results requires looking beyond whether a marker is simply flagged “high” or “low.”

Instead, we need to consider:

  • Your symptoms
  • Your medical history
  • Your diet
  • Your stress levels
  • Your digestive health
  • Trends over time rather than one isolated result

Blood tests should never be interpreted in isolation.

Understanding reference ranges

To understand why this happens, it helps to know how laboratories create reference ranges.

Most laboratories establish their ranges by measuring thousands of people within the population and determining where approximately 95% of those results fall.

The problem? Those people aren’t necessarily at optimum health.

Many may already have:

  • Chronic stress
  • Poor sleep
  • Nutrient deficiencies
  • Insulin resistance
  • Digestive issues
  • Low-grade inflammation

So being “within range” simply means you’re statistically similar to most of the population.

It doesn’t necessarily mean your body is functioning optimally.

Functional Analysis: Looking at the Bigger Picture

Functional blood test analysis doesn’t replace conventional medicine, it complements it.

Instead of asking whether you’re sick enough to receive a diagnosis, it asks:

  • Why are you experiencing symptoms?
  • Are there subtle patterns developing?
  • Could several “normal” markers together explain how you’re feeling?

Rather than focusing on one isolated number, I look for patterns across multiple biomarkers.

Sometimes the answers are hiding in plain sight.

Iron Studies Explained

Iron deficiency is one of the most commonly overlooked issues I see, particularly in women.

Many people assume that if their haemoglobin is normal, their iron status must also be normal.

Unfortunately, it isn’t that simple.

When assessing iron, I like to review the entire iron panel together:

Ferritin

Reflects your stored iron. Low ferritin is commonly associated with fatigue, poor exercise recovery, hair loss and restless legs, even when haemoglobin is normal. Ferritin alone can also be misleading – if there’s inflammation present for example.

Serum Iron

Measures the amount of circulating iron in your blood at the time of testing. It can fluctuate throughout the day and shouldn’t be interpreted on its own.

Transferrin

A transport protein that carries iron around the body. It can increase when iron stores are low.

Transferrin Saturation

Shows how much of the available transferrin is carrying iron. Low saturation may suggest reduced iron availability.

Looking at all of these markers together provides a much clearer picture than relying on a single number.

Why you can Feel Unwell Despite Normal Blood results

Blood tests are only one piece of the puzzle.

Symptoms can also be influenced by:

  • Poor nutrient absorption
  • Chronic digestive issues
  • Gut dysbiosis
  • Ongoing inflammation
  • Chronic stress
  • Poor sleep
  • Hormonal imbalances
  • Lifestyle factors

This is why I spend so much time listening to my clients before I even begin looking at their pathology.

When someone comes to me saying, “My blood tests were normal but I’m still exhausted,” I typically run a thorough panel rather than relying on a handful of standard markers.

Again, it’s rarely one marker alone that explains someone’s symptoms. It’s the combination of results and the story they tell.

Final Thoughts

One of the biggest lessons I’ve learned in practice is that numbers don’t exist in isolation. I never diagnose based on a single result.

Instead, I look for patterns, trends and connections between someone’s blood work, symptoms, lifestyle and overall health.
